Medication Administration Form
Dropping Off Parent-Provided Medications at School
To ensure the safe transport and handling of medications, all prescription and over-the-counter medications must be delivered directly to the clinic by a parent or guardian. Students may not carry medication at school unless written authorization is provided by a physician.
Over-the-Counter Medications
- Medications must be in their original packaging with the manufacturer's dosing information clearly visible.
- School nurses may administer only the dosage indicated on the manufacturer's label.
- If a healthcare provider has directed your child to take the medication differently than indicated on the label, a written prescription or medication order from the healthcare provider is required.
Prescription Medications
- Medications must be provided in the original prescription container with the student's name and current dosing instructions clearly displayed on the label.
- A written medication order from the healthcare provider must accompany the medication.
End-of-Year Medication Pickup
Any medication not picked up by a parent or guardian on or before the last student day of the school year will be properly disposed of.
