Dropping Off Parent-Provided Medications at School

To ensure the safe transport and handling of medications, all prescription and over-the-counter medications must be delivered directly to the clinic by a parent or guardian. Students may not carry medication at school unless written authorization is provided by a physician. 

Over-the-Counter Medications

  • Medications must be in their original packaging with the manufacturer's dosing information clearly visible.
  • School nurses may administer only the dosage indicated on the manufacturer's label.
  • If a healthcare provider has directed your child to take the medication differently than indicated on the label, a written prescription or medication order from the healthcare provider is required.

Prescription Medications

  • Medications must be provided in the original prescription container with the student's name and current dosing instructions clearly displayed on the label.
  • A written medication order from the healthcare provider must accompany the medication.

End-of-Year Medication Pickup
Any medication not picked up by a parent or guardian on or before the last student day of the school year will be properly disposed of. 

PHHS Medication Administration Form

Parents of PHMS and elementary students must obtain and sign the medication administration form in the nurse's clinic.
